Vidalista is a brand of tadalafil tablets made by Centurion Remedies, a manufacturer in Vadodara, Gujarat. The brand covers several strengths and the 40 is how much tadalafil is in one tablet.
Tadalafil belongs to a group of drugs called phosphodiesterase type 5 inhibitors, usually shortened to PDE5 inhibitors. Sildenafil and vardenafil are in the same group. What separates tadalafil from the others is how long it stays active in the body, which is why men who dislike planning around a tablet tend to end up on it.
It is a prescription medicine in most countries. That is not a formality. Tadalafil relaxes blood vessels throughout the body, which is exactly why it must not be combined with certain heart medicines, and why a doctor needs to know your blood pressure and what else you take before choosing a dose.
What tadalafil treats
Tadalafil is licensed for three different things. They use different doses and are worth keeping apart, because a lot of confusion about the 40 mg strength comes from mixing them up.
Erectile dysfunction
The main use, and the reason most people are reading this. Tadalafil helps a man get and keep an erection firm enough for sex, provided he is aroused. It does not create desire and nothing happens without stimulation.
Enlarged prostate
Benign prostatic hyperplasia causes a weak stream, trouble starting, and getting up at night to urinate. Tadalafil relaxes smooth muscle in the prostate and bladder neck and eases those symptoms. This is treated with a small dose taken every day, not with a 40 mg tablet.
Pulmonary arterial hypertension
A condition where the blood vessels between the heart and lungs are narrowed, which makes the heart work harder and leaves people breathless. Tadalafil widens those vessels and improves how far someone can walk. This is the one licensed use of a 40 mg daily dose, and it is managed by specialists with monitoring.
How it works
An erection is a plumbing event driven by a chemical signal. Knowing the four steps makes the rest of this page easier to follow.
- Arousal makes nerves in the penis release nitric oxide.
- Nitric oxide triggers production of a messenger called cGMP.
- cGMP relaxes the smooth muscle in the penile arteries, they widen, and blood flows in faster than it drains out. That is the erection.
- An enzyme called PDE5 breaks cGMP down again, and the erection subsides.
In erectile dysfunction that fourth step often wins too early, or there was never enough cGMP to begin with. Tadalafil blocks PDE5, so cGMP survives longer and the erection holds.
Two things follow from this. Tadalafil only amplifies a signal that arousal has already started, which is why it does nothing on its own. And because it works by keeping a pathway open rather than forcing one, there is a ceiling on what more of it can achieve once that pathway is already well supported.
PDE5 also sits in smooth muscle elsewhere, including the lungs and the prostate, which is how one drug ends up treating three unrelated looking conditions. It is also why tadalafil lowers blood pressure a little everywhere, and why combining it with other things that do the same is the central safety issue.
Where 40 mg sits
For erectile dysfunction, tadalafil is normally started at 10 mg taken before sex, then adjusted between 5 mg and 20 mg depending on how well it works and how well it is tolerated. Twenty milligrams is the top of the licensed range.
Forty is double that. The reason 40 mg tablets exist at all is the pulmonary hypertension dose, which is 40 mg once daily. Some doctors do prescribe 40 mg for erectile dysfunction when 20 mg has not produced a result, and that is a legitimate decision for a prescriber who knows the patient, but it is made outside the licence rather than according to it.
Doubling the tablet does not double the result. Trials mapped the response across 5 to 20 mg and did not establish extra benefit above it. What does keep climbing with the dose is the headache, the indigestion, the flushing and the aching back.
If a normal dose is doing nothing, that is usually information rather than a reason to take more. Diabetes, high blood pressure, high cholesterol, low testosterone, sleep apnoea, heavy drinking, smoking, some antidepressants and blood pressure tablets, and plain performance anxiety all cause erection problems, and none of them are fixed by a bigger tablet. Erectile dysfunction is also one of the earlier signs of narrowing arteries, often showing up years before chest pain, so it is genuinely worth investigating.

How to take it
Tadalafil taken as needed is swallowed whole with a glass of water, 30 to 60 minutes before sex. Never more than one dose in 24 hours.
Do
- Take the dose your doctor actually prescribed, not the one on the strip.
- Swallow the tablet whole. Do not crush, chew or break it.
- Allow at least half an hour. A light stomach is marginally faster, but food is not a real obstacle.
- Give it a fair trial. Several attempts over a few weeks is a more honest test than one bad night.
- Keep the box, so anyone treating you can see exactly what you took.
Do not
- Take a second tablet in the same day because the first seemed weak.
- Combine it with sildenafil, vardenafil or any other ED tablet.
- Take it with nitrates or poppers, under any circumstances.
- Turn a 40 mg tablet into a daily habit. Daily tadalafil uses much smaller doses.
- Share it, or take one that was prescribed for someone else.
What changes the right dose
Kidney or liver impairment, which slow clearance. Drugs like ketoconazole, itraconazole, clarithromycin and ritonavir, which push levels up sharply. Alpha blockers, which add to the blood pressure effect. Age, heart conditions and everything else you take. Every one of those pushes the sensible dose down rather than up, which is the practical argument against guessing with a high strength tablet.
Splitting the tablet
Halving a 40 mg tablet to reach 20 mg sounds tidy and works badly. Hand split tablets come apart unevenly, so one half can still land well above the range, and some tablets are not made to be divided. If 20 mg is what you need, get a 20 mg tablet.
Onset and duration
Most men find tadalafil starts working within 30 to 60 minutes. Some need longer, and a very large fatty meal with a lot of alcohol will blunt it. Claims of an effect within 15 minutes do not match how the drug is absorbed.
The long tail is what tadalafil is known for. In trials it was still outperforming placebo 36 hours after the dose, so a tablet taken on Friday evening can still be helping on Sunday morning. That flexibility is the main reason men choose it over sildenafil.
Thirty six hours is a window in which you can respond to arousal. It is not 36 hours of erection, and no medicine should produce that. An erection lasting more than four hours is a medical emergency called priapism and needs treatment the same day, because after several hours it starts causing permanent damage.
The long window comes from how slowly the body clears tadalafil, not from the size of the tablet, so a bigger dose does not extend it in any useful way.
Side effects
Most men who get side effects get mild ones that ease as the body adjusts. They are more common at higher doses, which is the main practical argument against taking more than you need.

Get help the same day for an erection lasting more than four hours, sudden vision or hearing loss, chest pain or fainting. Tell whoever treats you that you have taken tadalafil, because it changes what they can safely give you, particularly if they were about to reach for a nitrate.
The back and muscle ache is the one that surprises people, because it arrives a day later and does not feel connected to the tablet. Simple painkillers and time usually deal with it. If it happens every single time, that is a good reason to discuss a lower dose rather than to put up with it.
Drug interactions
Tadalafil relaxes blood vessels, so anything else doing the same adds up. One group is an absolute no.

Safety advice by condition
Whether tadalafil is suitable, and at what dose, depends more on the rest of your health than on the erection problem itself. These are the situations that change the answer.
Heart and blood pressure
Sex loads the heart and tadalafil lowers blood pressure. Generally avoided after a recent heart attack or stroke, in unstable angina, in uncontrolled high or low pressure, and in significant heart failure. Anyone on nitrates must not take it at all.
Liver disease
The liver clears tadalafil, so impaired function means it lingers and side effects are more likely. Doses are usually reduced, and in severe disease some regimens are not recommended. Needs a doctor who has seen your blood results.
Kidney disease
Reduced kidney function also slows clearance. Lower doses and longer gaps are the usual approach, and daily dosing is generally avoided in more severe impairment. A strong argument against a high strength tablet.
Eyes
NAION, where blood supply to the optic nerve is cut off, has rarely been reported with PDE5 inhibitors, more often where risk factors already exist. Anyone who has lost vision in an eye this way should not take tadalafil.
Diabetes
Not a reason to avoid it, and many men with diabetes do well on it, though long standing diabetes can make erectile dysfunction harder to treat. Worth a conversation, because diabetes usually brings blood pressure and heart issues with it.
Blood disorders
Sickle cell disease, myeloma and leukaemia raise the risk of an erection that will not resolve, as does an anatomical deformity of the penis. All need discussing before starting.
Women and pregnancy
Licensed for men. Not properly studied in women and not recommended in pregnancy or while breastfeeding. Tadalafil is used for pulmonary hypertension in women in specialist care, which is unrelated to this.
Age
For adults, never under eighteen. Older men are not excluded, but are more likely to have the heart conditions, kidney changes and interacting medicines that shift the sensible dose down.
Tell your doctor about any bleeding disorder, stomach ulcer, retinitis pigmentosa, or any other sexual problem such as premature ejaculation, which tadalafil does not treat.
Who should not take it
- Anyone taking nitrates in any form, including poppers, for any reason.
- Anyone taking riociguat.
- Anyone who has reacted badly to tadalafil before.
- Men with a recent heart attack or stroke, unstable angina, uncontrolled blood pressure, very low blood pressure or serious heart failure.
- Men who have lost vision in an eye through NAION.
- Men with severe liver or kidney disease, unless a specialist says otherwise.
- Women, and anyone under eighteen.
Your doctor also needs to know about any bleeding disorder, stomach ulcer, sickle cell disease, retinitis pigmentosa, or a penile deformity, and about any other sexual problem such as premature ejaculation, which tadalafil does not treat.
Missed dose, too much, and storage
There is no missed dose with an as needed tablet. You take it when you need it. Nothing is lost by skipping days.
Do not take a second tablet because the first did not seem to work. Wait, try again another day, and if it genuinely does nothing across several attempts, that is a conversation with your doctor rather than a reason to double up. Taking more than intended mostly produces a much worse headache and a larger blood pressure drop. Seek help if you feel faint, get chest pain, or have an erection lasting over four hours.
Store tablets at normal room temperature, somewhere dry, in the original strip. Keep them away from children and pets, out of the bathroom and out of a hot car. Check the expiry date. Do not decant them into an unlabelled container, because in an emergency nobody can then tell a doctor what was taken.
Things that make treatment work better
Erectile dysfunction is usually a circulation problem, and circulation responds to how you live. None of this replaces a tablet, but it changes how well the tablet works and what happens over the next ten years.
- Stop smoking. Smoking damages the lining of blood vessels, which is exactly the tissue an erection depends on. This is the single highest value change.
- Move regularly. Aerobic exercise improves erectile function measurably in trials. Brisk walking counts.
- Lose weight if you carry extra. Abdominal fat lowers testosterone and worsens insulin resistance, and both feed into ED.
- Get your blood pressure, cholesterol and blood sugar checked. All three are common causes, and all three are treatable.
- Sleep. Untreated sleep apnoea is a frequent and frequently missed cause.
- Cut back on alcohol. It works against both the erection and the tablet.
- Review your other medicines. Some blood pressure tablets, antidepressants and prostate drugs cause ED. Often there is an alternative.
- Take anxiety seriously. Performance anxiety keeps the problem going after the physical cause is handled. It responds well to treatment.

Men who want spontaneity generally prefer tadalafil. Men who get the back and muscle ache often do better on a shorter acting drug, because the side effect clears with the drug. Trying more than one is normal and reasonable.
Vidalista and Cialis
Both are tadalafil. Cialis is the original brand from Eli Lilly and Vidalista is one of many products made after the patent expired. At the same dose neither is inherently stronger, because it is the same molecule. What differs is price, packaging and which regulator has approved that specific product.

Two points are worth making about the higher end of the range. Can I take Vidalista 40 every day?

Should I take it with food or on an empty stomach?

Which medicines must never be combined with it?

The combination can drop blood pressure to a dangerous level. Riociguat, used for pulmonary hypertension, is the same. Never take two ED drugs together either.
What about alpha blockers, antifungals and HIV medicines?
They are not forbidden but they matter. Alpha blockers such as tamsulosin add to the blood pressure drop, so men on them are usually started low. Ketoconazole, itraconazole, clarithromycin and HIV protease inhibitors like ritonavir slow tadalafil clearance and push levels up sharply, which means more side effects from the same tablet. Rifampicin and some epilepsy drugs do the opposite and can make it barely work.
Can I drink alcohol with it?
A drink or two is fine for most men. Heavy drinking is the problem, because alcohol lowers blood pressure on its own and stacks with the dizziness and headache. It also makes erections harder to get, so it works against the tablet.
What about grapefruit juice?
Grapefruit interferes with the same liver enzyme as several of the interacting drugs, so large amounts can raise tadalafil levels. An occasional glass is unlikely to matter. Drinking it regularly while on tadalafil is best avoided.
Can I take it if I have diabetes?
Many men with diabetes take tadalafil and it works for them, although erectile dysfunction caused by long standing diabetes can be harder to treat. What if I have liver or kidney problems?
